Long Term Care Advanced Practice Provider (APP)

About the role

The Long Term Care Advanced Practice Provider (APP) delivers autonomous clinical care within skilled nursing and long‑term care facilities. Working closely with medical directors, nursing teams, and interdisciplinary partners, the APP helps improve resident stability and reduce avoidable hospitalizations.

Key responsibilities

• Perform comprehensive assessments and manage acute and chronic conditions common in LTC populations.
• Develop and adjust individualized care plans in collaboration with facility teams.
• Manage high‑risk LTC needs such as infections, wounds, CHF/COPD, dementia, and polypharmacy.
• Provide a consistent clinical presence supporting workflows, new admissions, and care transitions.
• Participate in interdisciplinary care planning, education, and quality‑improvement initiatives.
• Support regulatory readiness, documentation accuracy, and survey/audit preparation.
• Utilize independent clinical judgment and appropriately escalate care.

Required profile

• Advanced degree (MSN, DNP, MPAS, DMSc, or equivalent).
• Active state licensure and board certification (AANP, ANCC, or NCCPA).
• DEA and BLS certifications.
• Strong interpersonal communication and knowledge of clinical practice standards.
• 2+ years of clinical experience in long‑term care, skilled nursing, or geriatrics (preferred).
